Assessment of the Kinetic Trajectory of the Median Nerve in the Wrist by High-Frequency Ultrasound

چکیده
Carpal tunnel syndrome (CTS) is typically diagnosed by physical examination or nerve conduction measurements. With these diagnostics however it is difficult to obtain anatomical information in the carpal tunnel. To further improve the diagnosis of CTS, an attempt using 30 MHz high-frequency ultrasound to noninvasively detect the local anatomical structures and the kinetic trajectory of the median nerve (MN) in the wrist was explored. Measurements were performed on the right wrist of 14 asymptomatic volunteers. The kinetic trajectory of the MN corresponding to flexion (from 0° to 90°) and extension (from 90° to 0°) movements of the fingers were detected by a cross correlation-based motion tracking technique. The average displacements of the MN according to finger movements were measured to be 3.74 and 2.04 mm for male and female subjects, respectively. Moreover, the kinetic trajectory of the MN in both the ulnar-palmar and total directions generally follows a sigmoidal curve tendency. This study has verified that the use of high-frequency ultrasound imaging and a motion tracking technique to sensitively detect the displacement and kinetic trajectory of the MN for the assessment of CTS patients is feasible.

Study of Median Palmar Cutaneous Nerve Involvement in a Patient with Wrist Ganglion
Background: Few reports represent palmar cutaneous nerve neuropathy in the wrist due to a ganglion cyst. Case Presentation: A 25-year-old female was presented with a mass at volar side of the wrist and hypoesthesia at the base of thenar eminence. At ulrasonographic examination, a simple cyst was detected. Sensory response of palmar cutaneous branch of median nerve was absent at the affec...
متن کاملComparison of Ultrasound Findings of Carpal Tunnel Syndrome before and after Corticosteroid Injection
Background and Objective: Carpal Tunnel Syndrome is the most prevalent type of peripheral neuropathy due to neural entrapment. This syndrome is primarily diagnosed based on clinical symptoms, and it is subsequently confirmed by electro-diagnostic studies. Ultrasonography as a safe, available, and inexpensive method is also valuable in the follow-up of patients. This study aimed to evaluate the ...
متن کاملMotor amplitude of median nerve with stimulation in the palm a reliable indicator for axonal loss in carpal tunnel syndrome
Background and Aim: The present study aimed to use the median nerve compound muscle action potential (CMAP) amplitude by stimulation at the palm instead of abductor pollicis brevis (APB) needle electromyography (EMG) for determining a sign of axonal loss in patients with carpal tunnel syndrome (CTS). Methods and Materials/Patients: This study was performed on 180 patients with CTS referred to ...

Introduction: Even though there is a high prevalence of carpal tunnel syndrome patients, there are very few reliable papers on the study of normal values and changes in sensory and motor latency parameters of the median nerve in the wrist region on the basis of age of patients .This retrospective study was on the basis of 5 years data (1998 -2003) from the electro diagnostic department of Sh. متن کاملPrevalence of ulnar-to-median nerve motor fiber anastomosis (Riché-Cannieu communicating branch) in hand: An electrophysiological study
Background: Two main muscles studied in the hand for evaluation of median nerve injuries are opponens pollicis (OP) and abductor pollicis brevis (APB). However, Riché-Cannieu communicating branch (RCCB) may limit the use of these muscles in electrodiagnosis. This condition is confusing in the case of median nerve injuries. This study was conducted to evaluate the prevalence of RCCB. &...
